How to apply

You need to complete an application form - use one of the following options:

Option 1: Download the form in Microsoft Word® format

This will enable you to complete and return the form to us electronically. Click here to open the document in Microsoft Word® format (this will open in a new window), and then save it onto your hard disk. Once saved, you can complete the form on your computer (you do not have to be online), then email it to our Personnel Department at personnel@fpiom.com
